It is to protect your best interest and mine to provide you with an appointment, medical history assessment and signed consent. This allows me to provide you guidance, advice and services safely and securely.

I abide by all HIPAA federal laws that protect your sensitive health information.; including, but not limited to, electronic medical records, photography, care plans, etc. I will have you sign a consent for photography and a detailed record of our visits to be shared with your/baby’s medical provider, where applicable.

There will be no charge to cancel/reschedule an appointment more than 24hrs before appointment time.
Cancellation of any appointment between 0-24hrs of scheduled appointment time will result in a $50 charge to the credit card I require you enter upon booking.

Did you know many health insurance companies cover lactation support as a result of the Affordable Care Act? I have partnered with TLN (The Lactation Network) and Wildflower Health to network with many insurance providers.

If I am not in network with your insurance provider, I will supply you with an itemized Superbill to submit for reimbursement. I encourage you to call your insurance company to assess your coverage.
I accept Mastercard, Visa and HSA/FSA cards. I cannot, however, guarantee reimbursement from your insurance company.
A $50 TRAVEL FEE will apply if your location is outside 20 miles of Intervale. Distance is calculated from the Intervale Scenic Vista in Intervale NH. This applies to insurance covered visits as well as self pay clients.
Your credit card is required at appointment booking to be kept on file, for all clients including TLN/Wildflower clients.
